Winter Sports Registration is Open!

You can now register online for middle school sports using the link provided above. If this is your first time setting up an account on FinalForms, you will need to set up your account first. You will choose your user name and password. This is not something we can see so please make sure you keep track of your information. (It has nothing to do with your school student account or ID number). Once you set up your user name and password, you will receive an email with a link to confirm your information. Once you click on that link, you can then page through and answer the remaining questions. Please indicate which sport your are registering for. Registration for winter sports open on October 1st.

Sports Physicals

Blank copies of the OSAA physical/health history forms can be found on the Athletics page of the Seven Oak website or you can pick up a blank copy in the Seven Oak office. Remember that physicals are good for 2 years from the date of the appointment. You can upload a copy of your athletic physical to FinalForms, drop off a copy in the Seven Oak Middle School office, or scan and email a copy to our athletic secretary (andrea.marsh@lebanon.k12.or.us).

Notice to Parents and Guardians

Due to recent budget cuts across our district, we are facing resource constraints that directly affect our program capacity. Regrettably, this means we must limit our overall roster sizes, and cuts will likely be necessary during tryouts this year. We know how much time, effort, and heart our student-athletes invest in their sports, and we understand how difficult this news may be to hear. Please know this is not a decision made lightly, nor does it reflect a lack of appreciation for our students' dedication. Our coaching staff is committed to making the tryout process as fair and supportive as possible. Every student will be given an equal opportunity to showcase their abilities, and any roster decisions will be communicated with care and respect. Should a student not make a team this season, we will share information on alternative ways to stay involved, including intramural opportunities, team manager roles, or community sports options.

Dear Parent or Guardian of a Student Athlete:

Per board policy, in order to transport athletes from a school competition in a private vehicle, the designated adult must have a valid Oregon Driver’s License and provide proof of automobile liability that meets or exceeds the current requirements, and copies of these on file at the Athletics Office.

They must also have to have a criminal history check with the district and have filled out the Proof of Auto Liability Insurance (EEAE) Form. All this must be completed and on file at the Athletics Office before carrying other students (excluding one's own child).

For an athlete to ride home with an adult other than their own parent or guardian, they must also have the approval of their coach. (Some coaches want all team members to ride to and from games together on the bus.)

Current minimum limits are:

$25,000 per person and $50,000 per accident for bodily injury,
$10,000 per accident for property damage,
$25,000 per person and $50,000 per accident for uninsured motorist coverage, and
$10,000 per accident for personal injury protection.

Practices

Note: On Wednesdays students must leave campus at dismissal and return for practice or sign-up for the Boys and Girls Club Afterschool Program at SOK.
Practices are weekly Monday through Thursday, 4:00-5:00 (unless there is a meet scheduled)

Location: Bleachers by the baseball field

For pickup: Parents can use the gravel lot behind the school
